Instructions for submitting applications online to the City of Madison, Community Development Division



1.   This module is to be used by existing or new service providers for the City of Madison Community Development Division. If you are not a provider organization, please discontinue at this step.



2.   Ensure that you have unblocked your pop-up blocker in your browser, and that you have installed a free version of Adobe reader (download at www.adobe.com) to read PDF files.



3.   If you have not already done so, you will need to download application material (forms) which you will complete and submit using this module. Click here to "Download Application Forms"



4.   If you are already registered in the system, please have your user name and password ready. If you are new to the system and receive a token (sent via email), please have that ready for the next step in this process. If you have neither, please pay close attention to the directions provided in the next step to establish access.
